Provide individualized, therapeutic-based support to youth and young adults (YYAs) experiencing homelessness. Support YYAs in quickly securing safe and stable housing while connecting them to resources that promote long-term stability and overall well-being. Work closely with clients to create individualized service plans that emphasize self-sufficiency, community integration, and holistic care, tailored to each client’s unique needs and strengths. The Care Coordinator builds trusting relationships with YYAs and collaborates with service providers to connect clients to housing, employment, healthcare, and personal development opportunities. This role requires active collaboration and teamwork to meet client needs, achieve program goals, and ensure consistent adherence to policies and procedures. The role requires strong communication skills, cultural competence, persistence, and a passion for helping young people move from homelessness to stability and independence.
