About The Position

Posterity Group LLC is seeking a Nurse Practitioner (PT Care Coordinator) to staff The Naval Medical Clinic San Diego and associated locations. This role involves providing a full range of outcome-based nurse practitioner services, including clinical supervision, general screening, medical care, examinations for routine, acute and chronic conditions, immunizations, and patient diagnosis, treatment, and counseling. The Nurse Practitioner will collaborate with the healthcare team to coordinate patient care, promote preventive care and health maintenance, and educate patients and families. The position also requires recognizing conditions needing isolation, ensuring universal precautions, and attending staff meetings for case discussion, continuing education, and quality improvement. The NP will provide comprehensive inpatient, outpatient, and telehealth care for maxillofacial surgery patients, acting as a key continuity provider across the care continuum. Responsibilities include perioperative coordination, inpatient management, post-discharge follow-up, and patient education, with an emphasis on complex and multidisciplinary populations. The role prioritizes continuity and optimization of surgical outcomes and involves working closely with Maxillofacial Surgery residents to support patient care and clinical efficiency while preserving resident educational opportunities. The NP will participate in inpatient rounds, outpatient clinics, and multidisciplinary conferences, contributing to workflow optimization, quality and safety initiatives, outcomes tracking, and operational support of the Center of Excellence. Clinical activities will align with the mission, scope, and strategic priorities of the Maxillofacial Surgery service and Center of Excellence.

Requirements

  • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited Nurse Practitioner program
  • Graduate from an academic program, within the specialty field of nursing study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
  • Possess a current, active, valid, and unrestricted license to practice as a Nurse Practitioner in any U.S. jurisdiction
  • Eligible for hospital credentialing and privilege
  • Minimum of 5 years of advanced practice provider experience, including at least 3 years in a surgical or procedural subspecialty (e.g., maxillofacial surgery, ENT, plastic surgery, trauma, head and neck oncology preferred)
  • Demonstrated experience managing complex perioperative patients in inpatient and outpatient settings
  • Experience coordinating care for multidisciplinary and high-acuity patient populations
  • Ability to independently assess, diagnose, and manage patients within scope of practice
  • Experience with perioperative optimization, postoperative management, and complication recognition.
  • Proficiency in ordering and interpreting diagnostic studies and prescribing medications per institutional policy
  • Experience in care coordination, case management, or clinical program support
  • Demonstrated ability to collaborate closely with surgeons and multidisciplinary teams.
  • Experience overseeing or mentoring clinical support staff and contributing to efficient clinic operations.
  • Participation in quality improvement initiatives, clinical pathway development, or outcomes tracking
  • Strong clinical judgment, communication, and organizational skills.
  • Ability to function with a high degree of autonomy while maintaining collaborative practice
  • Proficiency with electronic medical records and digital health tools.

Responsibilities

  • Provide a full range of outcome-based nurse practitioner services in accordance with privileges granted by the MTF (e.g., clinically supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated).
  • Collaborate with and provide clinical guidance to members of the healthcare team in the coordination of patient care.
  • Diagnose and treat patients with common acute conditions, chronic illnesses, or minor trauma within accepted protocols, clinical practice guidelines, Nurse Practice Acts, and/or in consultation with a physician.
  • Promote preventive care and health maintenance including annual physical, positive health behaviors and self-care through both formal and individual education and counseling.
  • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Recognize conditions that require isolation and ensure universal precautions are used in all patient encounters.
  • Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement and safety.
  • Provide comprehensive inpatient, outpatient, and telehealth care for maxillofacial surgery patients as an embedded member of the Maxillofacial Surgery service and the Center of Excellence in Maxillofacial Restoration.
  • Serve as a key continuity provider across the care continuum, supporting surgeon-led care delivery while exercising independent clinical judgment within established protocols, collaborative practice agreements, and Maxillofacial Surgery–defined care pathways.
  • Assess, evaluate, and manage patients across preoperative, perioperative, and postoperative phases of care, with particular emphasis on complex and multidisciplinary populations.
  • Manage perioperative coordination, inpatient management, post-discharge follow-up, and patient education.
  • Work closely with Maxillofacial Surgery residents to support patient care delivery, continuity, and clinical efficiency, while preserving resident educational opportunities and scope of training.
  • Participate in inpatient rounds, outpatient clinics, and multidisciplinary conferences.
  • Contribute to workflow optimization, quality and safety initiatives, outcomes tracking, and operational support of the Center of Excellence.
  • Ensure clinical activities remain aligned with the mission, scope, and strategic priorities of the Maxillofacial Surgery service and Center of Excellence.
