Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities. PURPOSE Care Coordination is an integral component of the philosophy of care at Clifford Beers Community Care Center. The Child First Care Coordinator works collaboratively with the Child First Clinician as part of a dyadic team to deliver comprehensive, relationship-based, trauma-informed services to young children and their families in accordance with the Child First model. The Child First Care Coordinator partners with families to identify strengths, promote resilience, reduce barriers to care, and improve child and family well-being through family-centered, culturally responsive, home- and community-based services. The Care Coordinator collaborates closely with the Child First Clinician to address social determinants of health, strengthen the parent-child relationship, coordinate services across systems, and connect families with community resources that support healthy child development. Services include a combination of billable and non-billable activities based on each family's strengths, needs, priorities, and level of acuity. Services are delivered primarily in families' homes and community settings to maximize engagement, reduce barriers to care, and promote long-term success. As a member of the Child First interdisciplinary team, the Care Coordinator participates in reflective supervision, interdisciplinary case consultation, and ongoing professional development to ensure fidelity to the Child First model and delivery of high-quality, evidence-based services. This position requires a flexible work schedule, including three scheduled evenings per week until clinic closure at 8:00 p.m., to accommodate family availability and program needs. DCF Child First Program Differential: Employees assigned to the DCF Child First Care Coordinator position are eligible to receive a DCF Child First Program differential of $9,100 annually, paid in equal biweekly installments. Eligibility for this differential begins upon assignment to the position and is contingent upon continued funding for the DCF Child First program
