Requirements

  • Experience in a care coordination, case management, or homecare role
  • Strong verbal and written communication skills
  • Highly organized with excellent time management and follow-up abilities

Responsibilities

  • Serve as the primary point of contact for clients, families, and care team members
  • Develop and monitor individualized care plans to ensure client needs are being met
  • Address client concerns and escalate complex cases to the appropriate team members
  • Coordinate and manage caregiver schedules to ensure consistent coverage
  • Track and follow up on authorizations, plan changes, and service updates
  • Maintain accurate and up-to-date client records and documentation
  • Liaise between clients, families, caregivers, and internal teams to ensure seamless care delivery
  • Provide timely updates to all parties on care plan changes and service status
  • Support intake and onboarding of new clients as needed
