About The Position

Terros Health is seeking a Families FIRST - Care Coordinator for our Stapley Rd Health Center in Mesa, AZ. The Families FIRST program assists families involved with the Department of Child Safety. The Care Coordinator will work closely with DCS, the clinical team, and other providers to coordinate services and help families achieve reunification and self-sufficiency. This role involves monthly meetings to set and achieve goals, providing resources and skills to promote home stability, and requires professionalism, friendliness, organization, and compassion.

Requirements

  • An associates degree in a human service or behavioral health field, and at least three years of related experience including working with multi-problem families in the field of substance abuse, child welfare, or behavioral health; OR a bachelor’s degree or higher in a human service or behavioral health field.
  • Proficiency using the MS Office suite, particularly Excel, Word and Outlook.
  • Knowledge in use of Electronic Health Record (NextGen is a plus).
  • Excellent oral and written communication skills.
  • Working knowledge of the child welfare/child safety system.
  • Must have or be able to attain CPR/First Aid certification within 60 days of assuming role.
  • Must pass background check in accordance with Terros Health and DCS requirements (no probation or parole within the last 10 years).
  • Must be able to pass the Arizona Department of Child Safety Direct Service Central Registry Clearance without any disqualifying events.
  • Must have a valid Arizona driver’s license, be 22 years of age, with a minimum of 3 years driving experience and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role.
  • Must pass a TB Test.

Responsibilities

  • Conduct activities as indicated on the “New Client Checklist” upon assignment of a new patient.
  • Complete a Short-Term Plan with assessed patients to ensure access to AFF services within three business days after assessment completion.
  • Conduct meetings or conference calls with the Service Team (patient, family members, DCS worker, and other agencies) to discuss and finalize the Service Plan within 15 days of the comprehensive assessment.
  • Schedule and conduct monthly Service Plan reviews with the Service Team to monitor progress, identify completed goals, and update the Service Plan quarterly for submission to the DCS Specialist.
  • Participate in Child & Family Team (CFT), Adult Recovery Team (ART), Team Decision Making (TDM) Meetings, Substance Exposed Newborn Safe Environment (SENSE) Meetings, and DCS/JOBS Case Plan Staffing’s.
  • Upload required documents into DCS’ designated record keeping system within prescribed timeframes.
  • Monitor the random alcohol and drug testing plan for assigned patients per AFF Drug Testing Procedures.
  • Coordinate with the DCS Specialist on missed tests, positive tests, and communicate needed changes to the drug testing plan.
  • Provide concrete supportive services based on family needs for sobriety, including car repair, housing assistance, utility assistance, clothing, food, phone, etc.
  • Confirm that outreach and re-engagement activities are conducted prior to closing a case, in coordination with a Recovery Coach.
  • Complete required documentation in patient records, including Progress Notes, conforming to professional practice, state licensing regulations, and company procedures, while meeting Production Metrics.
  • Drive to various locations throughout the assigned territory to visit and provide services to patients.
