Under the supervision of the Program Director, the Care Coordinator is responsible for all aspects of coordinating care for ROWI clients. This includes coordinating individualized services such as orientation, transportation, and doctor's appointments, as well as managing schedules. The role ensures that programs and services are delivered at the highest standards, providing clients with the utmost level of care. The Care Coordinator is also responsible for client supervision, ensuring attendance at designated groups, facilitating assigned groups and activities, and conducting client orientations. This includes administering screenings and assessments, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, and driving clients as needed. The position also involves documenting all transportation and group activities in the EMR, administering and submitting documentation for client Urine Analysis, assisting with facility cleanliness, and coordinating daily client lunches. The Care Coordinator serves as a role model, maintains client confidentiality, complies with the dress code, completes tasks accurately and in a timely manner, and attends staff meetings. The role requires the ability to maintain clear boundaries, set limits, manage clients needing direct supervision, and interact professionally with clients and staff. All EMR documentation must be submitted within 24 hours.
