Genesis Community Homes is looking for a dynamic, hands-on Care Coordinator to drive excellence in healthcare coordination for the individuals we serve, improve staff development, and program quality. Requirements
- Strong knowledge of IDD, mental health, or TBI populations, 6400 regulations & ODP standards
- Proven leadership and coaching ability
- Highly organized, proactive, and solution-focused
- Comfortable working independently and making decisions
- Tech-savvy (Microsoft 365 Suite, HCSIS/EIM)
- Bachelor’s degree in Social Work, Human Services, or a related field, plus 4 years of experience, including 2 years of management experience OR Associate’s degree in Social Work, Human Services, or a related field plus 6 years of experience including 2+ years of management experience
- Valid driver’s license
- Willingness to be flexible with your schedule (some evenings/weekends as needed) and be on-call as a resource for residential supervisors
Responsibilities
- Coordinate all medical appointments, records, and medication tracking for our individuals and ensure follow-through on physician recommendations
- Manage and maintain MARs, Lifetime Medical Reports, Current Medications lists and HRST updates within specified timeframes
- Attend ISP meetings with accurate and up to date information
- Serve as key liaison with providers, pharmacies, specialists and families
- Lead Medication Administration & CPR training programs
- Conduct audits, observations, and ensure regulatory compliance
- Partner with leadership to strengthen staff performance
- Design training that builds independence for the individuals we serve
- Serve as a coach and mentor to Residential Supervisors and staff
- Drive compliance with PA ch.6400 & ODP regulations
- Lead program audits and improve systems
- Act as Certified Investigator / Incident Lead
- Help maintain safe, structured, therapeutic homes
