About The Position
Sentara Health is hiring a Full Time Care Coordinator for the Roanoke, VA area. This position requires travel for in person face-to-face assessments in Roanoke, VA. Days without assessments scheduled, the employee can work remotely. Applicants must be located in Roanoke, VA.
Requirements
- Bachelor's degree in Health & Human Services or a related field (Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation).
- 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience.
- Must be located in Roanoke, VA.

- Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services.
- Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services.
- Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation.
Responsibilities
- Conduct in-person face-to-face assessments in Roanoke, VA.
- Coordinate care for behavioral health patients.
- Navigate services and assist with care transitions.
