The primary work location for this role is at a new program at the Radisson hotel in Oakland. Training will occur at other locations in the Bay Area. This program is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals. Cardea Health’s transitional housing programs work with a variety of different referring partners across Alameda County. Our transitional housing programs provide supportive services including onsite nurse and caregiver supports, transportation to medical appointments, meals, housing support linkage, and more. The care coordinator will assist clinical staff and residents in the coordination of medical and behavioral health care.

Requirements

  • High School Diploma or equivalent
  • Must be authorized to live and work in the U.S
  • Must be at least 18 years of age
  • Must have personal, reliable transportation to and from work sites and shifts

Nice To Haves

  • Prior experience with homeless or incarcerated populations is desirable
  • Self-directed and able to work with minimal supervision
  • Detailed oriented and highly organized
  • Proficiency in Microsoft Outlook, Word and Excel

Responsibilities

  • Maintain a calendar of upcoming resident medical and behavioral health appointments.
  • Communicate upcoming appointments to clients, clinical staff, and transportation services.
  • Assist clinical staff and residents in the scheduling of client medical and behavioral health appointments
  • Support in keeping site operations organized, consistent and smooth.
  • Communicate across departments and roles as needed.
  • Communicate with local pharmacies regarding resident prescriptions.
  • At the direction of nursing staff request medication refills, deliveries, and transfers
  • Request relevant medical records of residents from hospitals and community clinics.
  • Transmit medical records and other authorized requested documentation to partner clinics to facilitate care coordination
  • Interface with resident healthcare coverage plans to avoid interruption in services and facilitate care
  • Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
  • Ensure adherence to infection control standards on site among staff and residents
  • Answer calls to nursing station and respond independently to when appropriate
  • Assist nursing staff with light clerical duties including filing and making copies
  • Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
  • Other duties and responsibilities as required
