About The Position
Segue Health, a faith-based medical practice, is seeking a compassionate Care Coordinator to join their team serving patients in the Franklin, TN area. This role focuses on coordinating post-discharge transitional care management. The position requires a blend of clinical knowledge, strong organizational abilities, and excellent communication skills.
Requirements
- Experience in the medical field.
- Knowledge of Microsoft Excel.
- Ability to learn EMR/EHR software.
- Must have transportation to and from local hospitals.
- Attention to detail.
- Organizational skills.
Nice To Haves
- Experience in social work, nursing, and/or home health.
Responsibilities
- Work with referring facilities to generate lists of patients eligible for Transitional Care Management (TCM) services.
- Visit referrals while in the facility to explain the TCM program and identify anticipated post-discharge needs.
- Log referrals and discharges daily.
- Create patient charts in the EMR system and upload necessary documents (face sheet, H&P, DC Summary).
- Conduct post-discharge calls to patients within 24 hours.
- Schedule in-home visits for patients, preferably prior to discharge.
