The Care Coordinator acts as the primary consumer advocate for the assigned program. This role involves staying informed about consumer progress and coordinating all services delivered to the consumer and their family, both internally and through external referrals. The position requires administering assessments, developing and reviewing service plans, documenting clinical services, conducting case management sessions, providing vocational education, attending external appointments on behalf of consumers, making referrals, preparing progress reports, coordinating care with treatment teams and external providers, developing discharge plans, and arranging transportation. The Care Coordinator also participates in case conferences, understands HIPAA and ethical standards, and completes all required training.
