Under the supervision of the Program Director, the Care Coordinator is responsible for all aspects of coordinating care for ROWI clients. This role involves coordinating individualized services including orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards, clients receive the highest level of care, and are supervised to ensure attendance at designated groups. This position also involves facilitating assigned groups and activities, conducting client orientations, administering assessments, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation, administering and documenting Urine Analysis in EMR, assisting with facility cleanliness, coordinating client lunches, serving as a role model, maintaining client confidentiality, complying with the dress code, completing tasks accurately and timely, attending staff meetings, and performing additional duties as assigned by management. The role requires the ability to maintain clear boundaries, set limits, manage clients needing direct supervision, and interact professionally with clients and staff. All EMR documentation must be submitted within 24 hours.
