About The Position
Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. We’re currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitating communication between providers, and ensuring individuals receive the appropriate care for their individual needs.
Requirements
- A Bachelor’s degree in social work, human services, or a related field is required.
Nice To Haves
- Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required.
- An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required.
Responsibilities
- Help coordinate service provision for providers.
- Facilitate communication between providers.
- Ensure individuals receive the appropriate care for their individual needs.
