The Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and schedules. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator is also responsible for client supervision, facilitating assigned groups and activities, conducting client orientation, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, administering and submitting client Urine Analysis documentation in EMR, assisting with facility cleanliness, and coordinating client lunch daily. The role requires serving as a role model, maintaining confidentiality, complying with the dress code, completing tasks in a timely and accurate manner, attending staff meetings, and managing clients when in need of direct supervision. All documentation in EMR must be submitted within 24 hours.
