Care Coordinator
About The Position
Traditional Home Care has an immediate opening for a Care Coordinator in their Non-Medical Division. This is a salaried position paid weekly, with an aggressive bonus plan and a generous benefits package. The company offers a non-corporate, family-owned atmosphere. The Care Coordinator is responsible for monitoring and supporting the daily activities of services provided to clients in accordance with state regulations and company policy, ensuring quality coordination of service. Traditional Home Care focuses on delivering medical and personal care to customers in their homes, with direct care workers assisting with Activities of Daily Living to help clients live independently.
Requirements
- Previous home health care or home care experience highly desirable
- High School or GED required
- Associate’s Degree preferred
- Staffing experience preferred
- Excellent verbal/written communication, time management and organizational skills
- Ability to effectively multi-task
- Effective problem solving skills
- Strong presentation and training skills
- Strong customer service skills
Responsibilities
- Provide superior customer service and quality home care
- Build lasting relationships with clients, referral sources, payers, and community organizations
- Coordinate client services and manage scheduling issues
- Manage and grow your client caseload
- Develop strong, communicative relationships with the team
- Interview, hire, and orient reliable and compassionate caregivers
- Function as primary support for daily EVV and payroll functions
