About The Position

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. The Care Coordinator serves as a bridge between prospective clients and treatment access. This role promotes HealthRIGHT 360 services, prioritizes client engagement, and works collaboratively to address barriers to care. The Care Coordinator actively partners with staff, Safe Harbor and El Camino Shelters, and community organizations to ensure sustained access to resources and treatment interventions. Acting as a liaison between HealthRIGHT 360, the provider network, and the broader San Mateo County community, the Care Coordinator conducts outreach at Safe Harbor and El Camino Shelters, will be on site to promote services, and connects prospective and enrolled clients to treatment and ongoing support. This role also provides direct services to clients in both group and individual sessions, either onsite, offsite, or within community and school settings.
