About The Position
Senior Health is looking for a Care Coordinator to join our team. This role is perfect for someone who enjoys working closely with providers, processing referrals and schedules visits while building relationships with patients and care teams. If you want a stable weekday schedule, a supportive team environment, and meaningful work with geriatric patients, we would love to meet you.
Requirements
- 2-3+ years of experience as a Care Coordinator or medical office coordinator
- Working knowledge of referral process
- Working knowledge of QuickBase or similar healthcare operations management software
- Reliable transportation and ability to travel to office in Salt Lake County (Woods Cross)
Responsibilities
- Assist providers with scheduling appointments
- Schedule diagnostic testing and follow-up appointments
- Complete telephone follow-ups with referring partners, patients and facilities
- Enter information into the EHR and assist with documentation
