About The Position

The Care Coordinator is responsible for all aspects of coordinating care for ROWI clients. This includes managing individualized services such as orientation, transportation, and doctor's appointments. The role ensures programs and services meet the highest standards, and clients receive excellent care. The Care Coordinator also supervises clients, ensures their attendance at designated groups, and may facilitate groups and activities. Key duties include conducting client orientations, administering assessments, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation, and managing client lunch schedules. The position requires serving as a role model, maintaining confidentiality, and completing tasks accurately and on time. Additionally, the Care Coordinator must maintain clear boundaries, interact professionally with clients and staff, and ensure all EMR documentation is submitted within 24 hours.

Requirements

  • Minimum of a high school diploma, Bachelor’s degree preferred.
  • Minimum of two (2) years driving experience required.
  • Minimum of 1-year working with kids in a therapeutic setting
  • Must be able to communicate with people from all educational and cultural backgrounds.
  • Must cope constructively with stressful situations.
  • Must demonstrate tact, initiative, resourcefulness, professional manner, enthusiasm, and non-judgmental attitude.
  • Documents clearly and efficiently.
  • Must have strong written communication skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills.
  • Must have a valid driver’s license and eligibility for insurance with agency’s vehicles.
  • Must have valid First-Aid & CPR Certification or be willing to obtain.

Nice To Haves

  • Experience with junior high or high school students is preferred.

Responsibilities

  • Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
  • Responsible for client supervision and ensuring they attend designated groups throughout programming.
  • Facilitate assigned groups and activities as designated by the Facility Manager.
  • Conduct client orientation for all admits on day of admission or assign to another staff if not available. Including UA screening, PHQ9 and other assessments as directed.
  • Ensure compliance with Joint Commission Standards.
  • Document shift reports and share pertinent information with the team.
  • Coordinate the transportation schedule for all clients and drive clients as needed.
  • Document all transportation and group activity in EMR.
  • Administer and submit documentation in EMR for client Urine Analysis.
  • Assist with facility cleanliness as needed.
  • Responsible for coordinating client lunch daily.
  • Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
  • Maintain the confidential nature of clients and related activities.
  • Comply with the dress code.
  • Complete tasks in a timely and accurate manner.
  • Attend designated staff and in-service meetings.
  • Any additional duties as assigned by the management team.
  • Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision. Maintains appropriate and professional interactions with clients and staff.
  • All documentation in EMR must be submitted within 24 hours.
