About The Position

The Care Coordinator at Devereux Pennsylvania Children's Services will impact the individuals we serve by acting as a liaison between the treatment team and external stakeholders including: client families/guardians, funding agencies, agency supports and involved child serving systems including family court. This role is located in West Chester, Pa. The center serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.

Requirements

  • Bachelor’s degree in Social Work or related discipline required.
  • Six months experience in Social Service preferred.
  • Proficient computer skills, mastery of Microsoft Office Applications, experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position).
  • Strong organizational skills.
  • Excellent oral and written communication skills.
  • Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines.
  • Valid driver's license must be maintained.
  • Ability to meet 100% mastery of Safe and Positive Approaches (S&PA) techniques.

Responsibilities

  • Welcome families/guardians to the Devereux Family, relaying expectations for family engagement/involvement while child is in care
  • Communicate with families/guardians the treatment and services being provided to their child
  • Participates in client treatment team meetings and reports on family engagement, supervised family visitation, family court, and care coordination activities that facilitate current treatment and discharge planning
  • Complete documentation related to family communication/engagement and care coordination activities, and discharge summaries
