This is a CCBHC grant-funded position for a Care Coordinator, ending September 29th, 2027. The role requires building rapport and engaging effectively with individuals from diverse backgrounds using a warm, compassionate, and person-centered approach. Strong interpersonal and communication skills are essential for establishing trusting relationships, providing exceptional customer service, and maintaining professionalism in a fast-paced behavioral health environment. The Care Coordinator will be responsible for client access to care triage, screening, and scheduling into services. They will ensure client care is coordinated between the client and all healthcare providers, collaborating with local agencies for continuity of care. The position also involves ensuring clients have access to needed services during transitions between levels of care, providing education and support for physical and behavioral health, developing strategies with clients to reach their health care goals, and assisting with internal and external warm hand-offs. Data entry into electronic health records and other databases, tracking and reporting client outcomes, and completing required documentation in accordance with Many Rivers policies are also key responsibilities. Effective communication with supervisors, therapists, medical providers, and other team members is crucial for quality continuity of care. Participation in in-service trainings, completing assigned trainings, and attending required meetings are also expected.
