About The Position

The ThriveKids Care Coordinator (LMSW/LCSW) role is part of the ThriveKids Student Wellness program at Children’s Hospital New Orleans. This community-based program aims to enhance healthcare and academic outcomes for students by collaborating with schools to increase access to healthcare services. The program involves staffing schools with healthcare professionals and facilitating direct patient care. The Care Coordinator will provide care coordination services to youth enrolled in ThriveKids, leveraging Children’s Hospital New Orleans' specialty and behavioral health services to address student health needs. A key responsibility includes developing a comprehensive guide to external/community behavioral health resources for families. This is a full-time position as a direct employee of Children’s Hospital New Orleans, with the option of a 10-month or 12-month work schedule (with corresponding compensation adjustments). The role serves as a communication liaison between medical providers, families, and school personnel, and facilitates re-entry meetings for students returning after extended absences due to crisis or hospitalization. Insights from this role will contribute to the programmatic development of ThriveKids behavioral health initiatives. The position offers opportunities for career development within Children’s Hospital New Orleans and may include providing student/patient counseling. Travel to individual schools may be required.

Requirements

  • Master’s degree from an accredited school in Social Work.
  • Licensed in the State of Louisiana.
  • Experience or training in providing services to children and families.
  • Excellent communication skills.
  • Ability to work both independently and in a team environment.
  • Ability to prioritize.
  • Clinical knowledge to assess the needs of youth enrolled in Thrivekids.
  • LMSW in clinical supervision.
  • Maintain current driver's license and car insurance.
  • Working knowledge of federal and state laws, policies and regulations relevant to schools.
  • Record of high performance in challenging situations, requiring problem-solving skills and good judgment.
  • Good time management, communication (oral and written), conflict management, organizational and planning skills.
  • Ability to maintain key community partnerships through excellent people skills, demonstration of accountability, and self-awareness.
  • Ability to work collaboratively with other professionals to obtain positive solutions.
  • Strong critical thinking skills.
  • Willingness to contribute to program development.

Responsibilities

  • Provide care coordination services to youth enrolled in ThriveKids.
