As a Care Coordinator you are the Communication Hub, the Emotional Support Co-Worker, and the Air-Traffic Controller for our nurses and caregivers in flight. Requirements
- Must be at least 18 years of age
- High school diploma or equivalent required
- Previous office or administrative experience required
- Strong communication and organizational skills
- Ability to multitask and stay organized in a fast-paced environment
- Professional, dependable, and team-oriented attitude
Responsibilities
- Coordinate daily caregiver schedules based on client care plans and staff availability
- Support and communicate with caregivers working in the field
- Complete client openings in the field
- Respond to client and employee questions or concerns and assist with resolution
- Maintain accurate documentation, records, and reports
- Assist with quality initiatives, staff communication, and training support
- Provide professional and courteous communication with clients, families, and team members
