About The Position

Stonington Institute, a division of Universal Health Services (UHS), is seeking a Care Coordinator. Stonington Institute provides substance abuse treatment options for adults in Southeastern Connecticut, including medical detox and residential addictions treatment. They also offer partial hospitalization and intensive outpatient treatment at their Groton clinic. The Inn at Trails Corner provides sober living accommodations for clients attending outpatient programs. Requirements

Bachelor’s degree required.

Nice To Haves

Three (3) years’ experience in the substance abuse/mental health treatment field preferred.

Responsibilities

Completes a Discharge Plan for each client admitted to the Detox Program, documenting progress daily on approved forms and assuring completion of Discharge Checklists, “Risk of Overdose” Form, Basis 32, and Satisfaction Survey for each client.
Assures appropriate ROIs are completed for referral agencies and others as required.
Ensure the Step-Down process is maintained to provide continuity of care.
Works collaboratively with clients and members of the treatment team to incorporate client needs and preferences and the recommendations of the multidisciplinary treatment team into the discharge planning process.
Acts as a liaison with community providers and respond to community inquiries related to discharge planning processes and procedures.
