Requirements

  • Bachelor’s degree in behavioral sciences required (Social Work, Psychology, Sociology, Mental Health, etc.)
  • Mental Health Professional (MHP) designation required.
  • Flexibility and availability to respond to crisis calls from clients required.

Nice To Haves

  • Knowledge of trauma-informed care practices preferred.

Responsibilities

  • Coordinates care for 10-18 clients, depending on Pathways Tier designation.
  • Meet with youth, families, and other supports in the youth’s life to coordinate client care.
  • Assists clients with accessing services to improve health, including healthcare, housing, education, nutrition, etc.
  • Provides de-escalation support during behavioral health crisis and assists clients to Mobile Crisis Response services if needed.
  • Completes assessments for clients to determine level of need.
  • Participates in multi-disciplinary staffing’s for assigned clients.
  • Completes documentation of all contacts and other required information in a timely and quality manner.
  • Develops, facilitates, and maintains child and family team meetings.
  • Utilizes IM+CANS to develop an individualized strength -based service plan.
  • Collaborates effectively with psychiatrists, primary care providers, MCO Care Coordinators and other community supports in coordinating client care.
  • Completes other duties as assigned.
