As a Care Coordination Outreach Worker / IBCCP , you will help connect patients with preventive screenings, follow-up care, health education, and community resources. Position Overview Under the direct supervision of the Care Coordination Outreach Supervisor , the Care Coordination Outreach Worker / IBCCP supports the appropriate delivery of healthcare services to Aunt Martha’s patients. This position enrolls eligible patients in the Illinois Breast and Cervical Cancer Program (IBCCP), coordinates medical follow-up for infants requiring additional screening, completes health-risk assessments, and provides outreach and referrals for medium- and high-risk patients. The role also schedules follow-up appointments for patients recently discharged from an inpatient hospital stay or emergency department visit. The ideal candidate is organized, dependable, comfortable communicating with patients, and able to manage multiple priorities while maintaining accuracy and attention to detail.
