Performs comprehensive assessments in the participant’s home or health care facility to assess need for community services. Determines eligibility for CCP, accurately administers and scores Mini Mental Status Exam (MMSE) and Determination of Need (DON) and collects needed financial information. Creates a care plan to meet the needs of the participant and caregiver. Demonstrates ability to authorize CCP services and coordinate referrals to other benefits/services available to the participant. Completes Choices for Care assessments for Nursing Home or Supportive Living Facility placement. Demonstrates knowledge of the various types of assessments, billing codes and documentation required. Accurately completes Medicaid applications when indicated. Advocates, intervenes and follows-up in a timely manner to ensure services remain in place and are appropriate to the changing needs of the participant. Documents and completes follow-up on critical events. Updates care plan and completes redeterminations as needed. Meets established timelines for assessments and critical event follow-up. Rotates weekend on-call duties as required by IDOA for weekend assessments. Completes paperwork timely and accurately. Responds to participant/family inquires and phone calls in a timely manner. Participates in departmental and all-agency staff meetings; offers constructive input to the group. Completes annual educational requirements for IDOA and Mercyhealth.
