About The Position

The Care Coordinator - Older Adult Care role involves performing comprehensive assessments in participants' homes or healthcare facilities to evaluate their need for community services. This position determines eligibility for the Community Care Program (CCP), administers assessments like the Mini Mental Status Exam (MMSE) and Determination of Need (DON), and collects necessary financial information. The Care Coordinator will create individualized care plans to meet the needs of participants and their caregivers, authorize CCP services, and coordinate referrals to other available benefits and services. This role also includes completing assessments for nursing home or supportive living facility placement, accurately completing Medicaid applications, and advocating for participants by intervening and following up to ensure services remain appropriate and in place. The position requires timely documentation, follow-up on critical events, updating care plans, and completing redeterminations as needed, while adhering to established timelines. The role also involves rotating weekend on-call duties for assessments and timely completion of all paperwork and participant/family inquiries.

Requirements

  • Minimum of 1 year experience in needs assessment and care planning.
  • RN or a BSN, or have a BA/BS degree in social science, social work or related field.
  • One year of program experience (defined as assessment, provision, and/or authorization of formal services), may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or
  • LPN with one year of program experience which is defined as assessment of and provision of formal services and/or authorizing service provision.
  • Approved CPR or BLS certification required within 90 days.
  • Successfully complete IL Department on Aging Care Coordinator Certification.
  • Maintain a valid driver’s license and reliable transportation with proof of vehicle insurance when requested.
  • Have and maintain an acceptable driving record per Mercy’s HR-01 Fleet Safety policy.
  • Basic PC/Laptop skills.
  • Knowledge of Microsoft Office, Internet Explorer, Google Chrome.
  • Excellent customer service skills.
  • Effective verbal & written communication.
  • Organized, able to multi-task in a fast-paced environment.
  • Able to sit, stand and walk intermittently.
  • Ability to lift/carry laptop and paperwork to/from office and homes/facilities.
  • Must be able to navigate various entrances to homes/apartments/facilities.

Responsibilities

  • Performs comprehensive assessments in the participant’s home or health care facility to assess need for community services.
  • Determines eligibility for CCP, accurately administers and scores Mini Mental Status Exam (MMSE) and Determination of Need (DON) and collects needed financial information.
  • Creates a care plan to meet the needs of the participant and caregiver.
  • Authorizes CCP services and coordinates referrals to other benefits/services available to the participant.
  • Completes Choices for Care assessments for Nursing Home or Supportive Living Facility placement.
  • Accurately completes Medicaid applications when indicated.
  • Advocates, intervenes and follows-up in a timely manner to ensure services remain in place and are appropriate to the changing needs of the participant.
  • Documents and completes follow-up on critical events.
  • Updates care plan and completes redeterminations as needed.
  • Meets established timelines for assessments and critical event follow-up.
  • Rotates weekend on-call duties as required by IDOA for weekend assessments.
  • Completes paperwork timely and accurately.
  • Responds to participant/family inquires and phone calls in a timely manner.
  • Participates in departmental and all-agency staff meetings; offers constructive input to the group.
  • Completes annual educational requirements for IDOA and Mercyhealth.
