The Care Coordinator II is responsible for coordinating multiple aspects of the youth’s and/or families’ care, providing the coordination and delivery of high-quality psychosocial care. This role works with other care providers and community partners to ensure that the overall needs of the client(s) are met. The Care Coordinator II may also be assigned to lead projects or mentor program staff. This position will be on the Monroe Region Youth ACT team, which serves children/youth with serious emotional disturbance who are returning home from inpatient settings or residential services, who are at risk of entering such settings, or whose needs have not been met in more traditional community-based settings. Referrals for Youth ACT must come from Children’s Single Point of Access (C-SPOA). This is a full-time (40 hours) position, primarily on a Monday-Friday day shift schedule, but hours will be flexible based on client need. There will also be on-call expectations for this position.
