The Care Coordinator will also advocate on behalf of the client with healthcare professionals, utilize evidence-based practices like Motivational Interviewing, Harm Reduction Techniques, and Trauma-Informed Care principles, and collaborate with hospital staff on Transitional Care Planning. Additionally, the position involves conducting outreach and engagement activities, accompanying clients to appointments, evaluating progress, arranging transportation, completing documentation, and attending team meetings and assigned training.
