About The Position

Frontier Behavioral Health is a non-profit organization with over 100 years of experience in Spokane, dedicated to providing evidence-based treatment for adults, youth, and seniors facing behavioral health challenges. As a Trauma-Informed Care Organization, we are committed to the belief that recovery is possible and that clients can lead fulfilling lives. We partner with Molina Community Health Plan of Washington, Action Health Partners, and Medicare DSNP plans to offer Care Coordination services to individuals with multiple chronic conditions, who are medically vulnerable, and at high risk for emergency care usage. The Care Coordinator engages with members, assesses their needs, provides education and support, coordinates care, and makes referrals to community resources to help members manage their conditions. A key responsibility is creating and reinforcing Health Action Plans to address social, chronic medical, and behavioral health conditions, as well as coordinating referrals to social services, mental health care, substance abuse treatment, and long-term support services. Most of the work is performed off-site, typically in members' homes.

Requirements

  • Bachelor’s degree in behavioral sciences with one-year related experience working in behavioral health care or a health care setting.
  • Must apply for a Counselor Agency Affiliated – Registered (CAAR) credential from the WA State Department of Health within 30 days of hire
  • Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.

  • Experience working with children is preferred but not required.

Responsibilities

  • Meet in person and over the phone with members assigned to caseload and maintain communication to initiate, develop and finalize the Health Action Plan (HAP).
  • Screen, assess and provide brief interventions for common mental and substance abuse disorders, providing patient education about available treatment options.
  • Complete required periodic health and behavioral screenings.
  • Use decision support tools and registries to coordinate services and care across different settings and types of providers, identifying opportunities to streamline or otherwise improve services and eliminate duplication.
  • Monitor members (in person or by telephone) for changes in symptoms and treatment side effects or complications and link with appropriate care providers when necessary.
  • Provide brief intervention using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
  • Document patient progress and treatment recommendations.
  • Routinely assess progress with members Health Action Plans and revise to include new goals or changes in current goals.
  • Facilitate referrals for indicated social and specialty services (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
  • Complete core training curriculum and participate in regular training and technical assistance activities for health home care coordinators.
  • Comply with all contract requirements and FBH policies and procedures.
  • Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.
  • Requires travel to see clients in the community and travel between work sites.
  • Other duties may be assigned.
