About The Position

Koinonia Family Services is seeking a part-time ECM Care Coordinator for their enhanced care management (ECM) program in Tustin, serving the greater Orange County area. This role involves connecting individuals, particularly foster and at-risk youth and their families, to services and resources that meet their unique needs. Responsibilities include conducting assessments, developing individualized care plans, overseeing service coordination and referrals, and providing education for healthy lifestyle decisions. The Care Coordinator utilizes motivational interviewing to engage individuals in managing chronic conditions and removing barriers to health, social services, and behavioral health care. Bilingual skills in English and Spanish are strongly preferred. This is a caseload-driven position with initially 20-25 hours per week, with potential for increase or decrease based on client assignments and business needs. Shifts are generally during normal business hours but may extend into the early evening, with some scheduling flexibility available.

Requirements

  • A strong desire to help people in need achieve successful outcomes
  • Ability to speak Spanish
  • Minimum 2 years of case management (or related) experience
  • Knowledge of community resources
  • Excellent interpersonal and communication skills
  • Comfortable talking to people and motivating them to seek services
  • Ability to collaborate with interdisciplinary teams
  • Experience with high-needs populations
  • A high degree of integrity, ethics and confidentiality
  • Demonstrated ability to work independently and maintain productivity with minimal supervision
  • Cultural sensitivity and awareness
  • Valid driver’s license and auto insurance
  • High school graduate or GED required

Nice To Haves

  • Bilingual skills in English and Spanish are strongly preferred
  • Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus
  • Proficiency in electronic health records a plus

Responsibilities

  • Conduct assessments of individuals' needs and formulate individualized care plans
  • Educate and enroll clients about services, and motivate them to engage in services
  • Meet with individuals in their homes to facilitate services
  • Oversee and ensure the seamless coordination of services, including physical, mental, dental care and social services
  • Act as the primary liaison between individuals and their healthcare providers, and social services
  • Stay informed about community resources and actively refer individuals to appropriate services as needed
  • Maintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomes
  • Advocate for the needs and rights of clients within healthcare and social service settings
  • Provide clients and families with resources and guidance on making informed health and lifestyle choices
  • Document services according to ECM program standards
  • Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery
