About The Position

Koinonia Family Services is seeking a part-time Enhanced Care Management (ECM) Care Coordinator in Los Angeles (Gardena) to join their team. This role focuses on connecting individuals, particularly foster and at-risk youth and their families, to services and resources that meet their unique needs. The Care Coordinator will conduct assessments, develop individualized care plans, coordinate services, and provide education to promote healthy lifestyle decisions. Bilingual skills in English and Spanish are strongly preferred. This position utilizes motivational interviewing to engage individuals in managing chronic conditions and removing barriers to health, social services, and behavioral health care. The role is caseload-driven, with initial hours of 20-25 per week, potentially increasing based on client assignments. Scheduling offers some flexibility, generally during business hours but may extend into early evenings, provided client and business needs are met.

Requirements

  • A strong desire to help people in need achieve successful outcomes
  • Ability to speak Spanish
  • Minimum 2 years of case management (or related) experience
  • Knowledge of community resources
  • Excellent interpersonal and communication skills
  • Comfortable talking to people and motivating them to seek services
  • Ability to collaborate with interdisciplinary teams
  • Experience with high-needs populations
  • Proficiency in electronic health records a plus
  • A high degree of integrity, ethics and confidentiality
  • Demonstrated ability to work independently and maintain productivity with minimal supervision
  • Cultural sensitivity and awareness
  • Valid driver’s license and auto insurance
  • High school graduate or GED required
  • Must complete a criminal background clearance through the California Department of Justice and FBI
  • Must complete a pre-employment physical and TB test
  • Reliable transportation as driving is an essential job function

Nice To Haves

  • Bilingual skills in English and Spanish are strongly preferred
  • Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus

Responsibilities

  • Conduct assessments of individuals' needs and formulate individualized care plans
  • Enthusiastically educate and enroll clients about services, and motivate them to engage in services.
  • Meet with individuals in their homes to facilitate services
  • Oversee and ensure the seamless coordination of services, including physical, mental, dental care and social services
  • Act as the primary liaison between individuals and their healthcare providers, and social services
  • Stay informed about community resources and actively refer individuals to appropriate services as needed
  • Maintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomes
  • Advocate for the needs and rights of clients within healthcare and social service settings
  • Provide clients and families with resources and guidance on making informed health and lifestyle choices
  • Document services according to ECM program standards
  • Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery.
