About The Position

The Mount Sinai Adolescent Health Center (MSAHC) aims to eliminate racial, social, and economic obstacles to healthcare and wellness for young people by offering high-quality, comprehensive, integrated, and confidential services to all individuals who visit the Center or school-based health facilities at a low cost. The MSAHC promotes adolescent and young adult health as a national priority through its leadership in clinical care, specialized training, innovative research, and advocacy. The Care Coordinator will deliver trauma-informed services grounded in a health equity framework, ensuring culturally and developmentally appropriate care for at-risk adolescents (ages 10-26) and their families within MSAHC's integrated medical and mental health primary care setting. This role involves assessing and addressing patient needs, particularly those related to accessing essential support such as housing, food, legal, and educational assistance. The Care Coordinator will collaborate closely with patients to identify support systems and solutions, empowering them in their care decisions and working to establish measurable, achievable, and individualized goals while managing any emerging challenges. This position emphasizes a holistic approach to care, integrating behavioral, medical, and social support to tackle mental health issues, substance use, psychosocial difficulties, and health-related social needs that influence patient outcomes. The Care Coordinator will also involve families and caregivers as needed to support patient follow-through and healthy adolescent development. All services provided are trauma-informed and guided by a health equity framework, promoting culturally and developmentally appropriate care for diverse adolescent populations.

Responsibilities
