About The Position

The Care Coordination Technician supports patients and families by identifying needs, coordinating resources, and assisting with access to healthcare, community, and social support services. The position serves as an advocate and liaison between patients, families, healthcare providers, and external agencies to help address barriers to care and support continuity of services. The Care Coordination Technician also assists with designated patient screenings, discharge needs, resource coordination, and follow-up while providing professional, compassionate, and patient-centered service. The care coordinator technician operates within the framework of the Driscoll Children’s Hospital mission, vision, and values.

Requirements

  • Must have a high school diploma or equivalent
  • Experience or knowledge of Social Security Administration SSI/SSDI programs.
  • Medical Terminology preferred.
  • Prior medical office or hospital experience preferred.
  • Basic Microsoft Office skills required.
  • Ability to understand complex situations and interpersonal dynamics to effectively handle escalated customer and co-worker needs.
  • Requires a well-organized individual with an excellent capacity for effective time management.
  • Demonstrates ability to establish and maintain effective working relationships with the provider office staff and peers.
  • Demonstrates ability to operate personal computer programs.
  • Excellent communication skills

  • Experience with Texas Health and Human Services Commission (HHSC) or the Department of Aging and Disability Services (DADS) a plus.

Responsibilities

  • Maintains confidentiality and complies with hospital policies, procedures, regulatory requirements, and corporate compliance standards.
  • Provides professional, compassionate, and customer-focused service to patients, families, staff, and community partners.
  • Assesses basic patient and family needs and assists with connecting patients and families to appropriate hospital, community, state, and federal resources.
  • Coordinates resources and services related to transportation, lodging, meals, basic needs, discharge planning, and other identified barriers to care.
  • Serves as a liaison between patients and families, clinical staff, hospital departments, insurance providers, and community agencies to facilitate timely access to services.
  • Assists with discharge planning needs, including coordination of basic durable medical equipment and required documentation to support a safe and timely discharge.
  • Maintains knowledge of hospital and community resources and develops professional relationships with community agencies to facilitate effective referrals and service coordination.
  • Completes appropriate documentation, tracking, and reporting related to services provided and maintains accurate records.
  • Supports designated patient screenings, resource referrals, and follow-up activities within established guidelines and timeframes.
  • Answers departmental calls and appropriately routes inquiries while identifying and escalating patient or family concerns as needed.
  • Maintains proficiency in required computer applications and department systems and participates in ongoing training and cross-training to support department operations.
  • Participates in departmental meetings, hospital committees, quality initiatives, special projects, and other activities as assigned.
  • Provides operational support for department programs and services, including resource management, inventory, tracking, reporting, and other administrative functions as needed.
  • Performs other duties as assigned and serves as a flexible resource to support patient and family needs across the department.
