Requirements

  • Master's degree in related field is required.
  • Experience in mental health and facilitating groups.
  • Bilingual proficiency in English and Spanish, with excellent verbal and written communication skills.
  • Proficiency with electronic health record systems; experience with the AWARDS system and internet-based applications is preferred.
  • Willingness to participate in trauma-informed care training and implement trauma-informed interventions.

Responsibilities

  • Supervise the day-to-day activities of the Care Coordination team.
  • Participate in individual and group supervision sessions led by the Program Director.
  • Lead team meetings with Care Coordinators to support the implementation of goals, care plans, assessments, HARP services, core Health Home services, and timely documentation.
  • Support staff development through training, coaching, and team-building activities.
  • Utilize electronic health record systems and databases required by the Health Home program.
  • Maintain a caseload, providing clinical expertise and stabilization for designated individuals, including Assisted Outpatient Treatment (AOT) participants transitioning to Care Coordination services.
  • Address client concerns and support problem-solving throughout enrollment in the Health Home program.
  • Provide client-centered, goal-oriented supervision focused on achieving measurable, data-driven outcomes.
  • Monitor client records and documentation to ensure compliance with Health Home standards, regulatory requirements, and agency policies.
  • Conduct individual and group clinical supervision sessions with Care Coordinators.
  • Facilitate case conferences and oversee incident reporting and follow-up procedures.
  • Communicate important follow-up items and provide ongoing guidance to Care Coordinators as needed.
  • Prepare and submit all required weekly, monthly, and other program reports in a timely manner.
  • Demonstrate strong interpersonal, written, and verbal communication skills to promote staff development and effective service delivery.
  • Perform other duties and special projects as assigned by the Program Director or Senior Director.
