Requirements

  • Must be an RN, or a BSN, or have a BA/BS degree in social science, social work or related field.
  • One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree.
  • Experience working with older adults and/or caregivers required.
  • Ability to relate well to persons with disabilities and older individuals.
  • Ability to work effectively in a dynamic, fast-paced, and diverse environment.
  • Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  • Strong verbal and written communication skills.
  • Passage of complete background clearance.
  • Valid driver’s license, reliable transportation, and proof of liability insurance.
  • Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook.

Nice To Haves

  • Master’s Degree preferred but not required.
  • Prior supervisory experience preferred.

Responsibilities

  • Organize, coordinate, and supervise day-to-day operations of the Care Coordination Unit.
  • Provide direct supervision and oversight to Care Coordination Staff including hiring, onboarding, training, performance management, and evaluation.
  • Provides clinical support and case consultation to Care Coordinators, advising on challenging cases.
  • Ensure education, training, and recertification of all assigned staff.
  • Support clients in the Community Care Program in the absence of regular Care Coordinator staff and ensure programmatic coverage, otherwise.
  • Maintain cooperative relationships with service providers and community organizations.
  • Create and maintain confidential case records, ensuring compliance with program standards.
  • Complete all required reporting and documentation accurately and timely.
  • Ensures effective outcomes and appropriate outputs for program.
  • Act as an agency ambassador in communities served, maintaining strong knowledge of available programs.
  • Actively support and hold each supervised employee responsible for achieving goals related to job performance, professional growth and development goals, fostering a culture of continuous learning and accountability.
  • Other duties as assigned within the guidelines of this position.
