Provides intensive and professional case management services to youth utilizing a strength based and comprehensive model. Links children and families to resources and supports, monitors the services, actively advocates for children and families, and assists them in navigating through various systems of care. Duties include providing safe and effective services, adhering to policies related to safety and boundaries, attending safety and abuse risk management training, adhering to procedures for managing high-risk activities and supervising service recipients, reporting suspicious or inappropriate behaviors and policy violations, and following mandated abuse reporting requirements. Case management services involve developing and maintaining effective working relationships with families and community resources, providing assessment to determine program eligibility and areas of strength, maintaining current knowledge of available community resources, identifying needed services and linking youth to them, identifying informal supports and assisting youth and family members to access them, monitoring linked services to ensure quality of care, acting as a youth and family advocate, and empowering youth and family members to advocate for themselves. The role also involves assisting youth and family members in navigating various systems, educating them about these systems, supporting permanency planning and outcomes, providing support in the development and utilization of functional skills, developing, monitoring, and reviewing service plans, maintaining direct contact with youth and family members, and being available for crisis intervention. Discharge planning, including step-down placements, reunification, adoption, or independent living plans, and coordination of community resources are also key responsibilities. Additionally, the role assists youth and family members in building on their spiritual and religious beliefs as a source of strength and promotes a culture of professionalism. Travel to various locations, including client homes, is required, utilizing one's own vehicle. Administrative functions include preparing and submitting reports on caseload status, budget requests, service plan changes, and unusual incidents, reporting critical incidents, and completing timely documentation such as case notes, assessments, service plans, and outcome measures. Feedback from supervisors is implemented, and compliance with contractual, regulatory, program, and accrediting body standards is ensured. Notification of any changes in professional status or standing that may affect job status or program compliance is required. There is a possibility of becoming a certified Common Sense Parenting Instructor and conducting related courses.
