About The Position

The Registered Nurse for Care Coordination concurrently and retrospectively ensures the patient's progress in the acute episode of care through post discharge, focusing on quality, efficiency, cost-effectiveness, and safety of the discharge plan. This role involves working with physicians to facilitate effective and efficient transitions through the hospitalization process. The RN collaborates with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, ensuring appropriate reimbursement by payers and efficient utilization of resources. Additionally, the RN serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.

Requirements

• Bachelor's Degree in Nursing, or related field
• Basic Life Support Training
• Registered Nurse in Iowa or Illinois at time of hire
• Second state (Iowa or Illinois) RN licensure required within 1 year
• More than 3 years experience

Nice To Haves

• Certification in Case Management, Professional Utilization Review or Managed Care
• DRG/CPT knowledge
• Interqual Admission/Continued Stay criteria knowledge
