About The Position

Eastern Area Agency on Aging is currently seeking a Care Coordination Specialist to join our Community Services team. This is a dynamic and rewarding role helping older adults, individuals with disabilities, and caregivers navigate services, maintain independence, and improve quality of life. This is a full-time, 40-hour-per-week position supporting our Veterans Directed Care and Money Minders programs. As a Care Coordination Specialist, you will: Conduct intakes and assessments over the phone and on home visits Develop person-centered care plans and coordinate services and supports Connect individuals to in-home and community-based resources, benefits, and programs Provide ongoing support, follow-up, and advocacy for consumers and their families Maintain accurate documentation and budgets and manage program requirements Build strong partnerships with community agencies and service providers Participate in outreach, education, and community engagement efforts This position includes regular home visits throughout Penobscot, Piscataquis, Washington, Hancock, Waldo, and Knox counties. Initial orientation will occur in our Brewer office, after which the position can be fully remote based in one of EAAA's primary service areas (Penobscot, Piscataquis, Washington, or Hancock county).

Requirements

  • Bachelor’s degree in Social Work, Human Services, Human Development, Gerontology, or a related field required; LSW or eligibility for licensure preferred
  • 2–3 years of experience in assessment, service planning, or care coordination
  • Strong communication, organization, and computer skills required; experience with documentation and database systems preferred
  • Ability to manage multiple responsibilities and work both independently and collaboratively
  • Must have a valid driver’s license, proof of vehicle insurance, and ability to travel throughout service area for job-related requirements.
