About The Position

Health Solutions is a premier wellness center focused on whole person care. With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. We’re looking for an Integrated Healthcare Case Manager to join our Care Coordination team at The Perry Hall in Pueblo.

Requirements

  • Baccalaureate degree in social work, clinical psychology, healthcare management, human services, or a closely related field
  • Computer proficiency
  • Colorado driver’s license, a good driving record, and access to transportation during work hours

  • Experience successfully working in multi-disciplinary teams
  • Experience working with adults with severe and persistent mental illness
  • Experience providing case management services to adults
  • Community systems navigation background
  • Spanish language skills
  • CAC training
  • Experience working with those experiencing co-occurring disorders
  • Knowledge of and experience with HUD housing rules and regulations

Responsibilities

  • Coordinate and support patient care activities to promote continuity, access, and quality outcomes across services
  • Assess and respond to patient needs by addressing clinical, behavioral, and social factors that may impact health and well-being
  • Facilitate connections to appropriate internal services and community resources to support comprehensive care
  • Support patient engagement and access to services, helping to ensure timely utilization of care and adherence to treatment plans
  • Collaborate with interdisciplinary teams and external partners to ensure coordinated, integrated service delivery
  • Serve as a point of communication between patients, providers, and community organizations to support continuity and alignment of care
  • Contribute to the achievement of organizational goals, performance metrics, and quality initiatives
  • Maintain accurate and compliant documentation in accordance with organizational, regulatory, and professional standards
  • Participate in cross-functional activities and team initiatives to support operational needs and program effectiveness
  • Assist in the development and improvement of workflows, processes, and best practices to enhance efficiency and service delivery
  • Identify opportunities for improvement and support problem-solving, innovation, and quality enhancement efforts
  • Foster effective communication and collaboration across departments to support a team-based approach to care and operations
  • Perform additional duties as assigned to support program growth, patient care, and organizational priorities
