Requirements

  • High School/GED.
  • Must be bilingual, demonstrating the ability to speak both English and one of two target languages - Spanish or Amharic.
  • Must be able to perform the responsibilities of the position either in English or in another language.
  • 1 to 3 years' experience in a health-related setting.
  • Proficient PC skills in a Windows based environment including word processing, spread sheets and working in database programs.
  • Working knowledge of basic healthcare terminology; especially for maternity population.
  • Excellent telephone/customer service skills.
  • Proven ability to keep accurate and timely records and documentation according to established processes.

Nice To Haves

  • Working knowledge of Plan Benefits and Services is preferred.

Responsibilities

  • Supporting the daily operations of integrated care management program interventions.
  • Assisting members with Care Gaps/HEDIS related health conditions in accessing care through Plan benefits and community resources.
  • Providing members with educational materials and carrying out strategies to increase health care adherence and reduce barriers to care, under the direction of Clinical staff.
  • Managing a caseload of members with low-risk health concerns who require assistance with navigating health systems.
