Requirements

High School/GED.
Must be bilingual, demonstrating the ability to speak both English and one of two target languages - Spanish or Amharic.
Must be able to perform the responsibilities of the position either in English or in another language.
1 to 3 years' experience in a health-related setting.
Proficient PC skills in a Windows based environment including word processing, spread sheets and working in database programs.
Working knowledge of basic healthcare terminology; especially for maternity population.
Excellent telephone/customer service skills.
Proven ability to keep accurate and timely records and documentation according to established processes.

Nice To Haves

Working knowledge of Plan Benefits and Services is preferred.

Responsibilities

Supporting the daily operations of integrated care management program interventions.
Assisting members with Care Gaps/HEDIS related health conditions in accessing care through Plan benefits and community resources.
Providing members with educational materials and carrying out strategies to increase health care adherence and reduce barriers to care, under the direction of Clinical staff.
Managing a caseload of members with low-risk health concerns who require assistance with navigating health systems.
