About The Position

As a Care Associate in the Hematology Clinic, under the direction of a Registered Nurse and as a member of the interdisciplinary healthcare team, you will provide patient-centered care based on respect for patient preferences, values, and needs. This role involves direct and indirect patient care activities crucial for delivering high-quality, safe patient care and an exceptional patient experience. The Unlicensed Assistive Personnel (UAP) is expected to demonstrate competence in patient-centered care, teamwork, communication and collaboration, evidence-based practice, safety, and informatics, as defined by their scope of practice. Patient and family centered care (PFCC) at YNHH is emphasized, adhering to its four principles: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence through exemplary customer service for all groups, including patients, families/friends, physicians, staff, and support personnel, while adhering to the 'I Am YNHH' service excellence pledge and promoting a positive work environment.

Requirements

  • A minimum of a high school diploma or GED.
  • Graduated from an approved, accredited, postsecondary medical assisting program, Nursing Assistant Program, or Patient Care Associate Training.
  • Demonstrated ability to communicate effectively with patient, families, peers, and interdisciplinary team.
  • Demonstrate flexibility in adjusting priorities and adjust work plans based on the needs of the patient, the service/unit, and the patient care team.
  • Demonstrate professional, appropriate, effective and tactful written, verbal and nonverbal communication with patients, families, medical staff, colleagues, and all internal and external employees.
  • Able to lift a minimum of 40 pounds.
  • Ability to perform tasks of physical endurance and dexterity as required by placement.
  • Consistent use of proper body mechanics.

Nice To Haves

  • 12-18 months experience prior work as a Medical Assistant, Nursing Assistant, or Patient Care Associate, preferably in an outpatient clinical setting or relevant patient care environment.
  • Clinical competency.
  • Current medical assisting credential (certifications or registration) acceptable to, and recognized by, the Connecticut Department of Public Health.
  • Blood draw experience.
  • EKG experience.

  • Practice behaviors specific to High reliability organizations (HRO) i.e. CHAMP behavior.
  • Demonstrate effective use of equipment that support quality and safe patient care.
  • Use information and technology to communicate information, mitigate error, and support team decision making.
  • Achieve and maintain competencies in clinical applications to support areas of responsibilities.
  • Demonstrate accountability for clinical competency in care delivery.
  • Demonstrate accountability and responsibility for own practice.
  • Perform clinical functions as per scope of practice, including: Appropriately identifying patient, identifying self to patient/family and explaining the role of the UAP, escorting patients to exam/procedure room, preparing patients for visit using standardized rooming workflow/protocol (e.g., vital signs, height, weight, blood pressure, EKG, specimen collection, phlebotomy, INR, pulse oximetry, point of care testing).
  • Perform clerical, administrative functions as per scope of practice, such as sending/receiving patient medical records and abstracting new patient records into EPIC.
  • Perform environmental support functions as per scope of practice, such as setting up instruments and equipment according to department protocol.
