About The Position

Cogir Senior Living is seeking a Care Assessment Coordinator (RPN Required) to join their team. This role is responsible for completing clinical assessments for prospective residents in various settings, including their homes and hospitals. The Care Assessment Coordinator will work closely with Lifestyle Consultants to support new business development by ensuring timely and accurate assessments and guiding families through the transition to retirement living. This position does not provide on-site nursing care. The goal is to ensure timely completion of clinical assessments, increase conversions and occupancy through strong partnerships, and provide a positive experience for families making care decisions.

Requirements

Registered Practical Nurse (RPN) in good standing
Strong assessment, communication, and interpersonal skills
Self-motivated, organized, and comfortable working independently
Passion for supporting seniors and their families

Nice To Haves

Experience in geriatrics, retirement living, or community care preferred

Responsibilities

Conduct clinical assessments for prospective residents in hospitals, homes, and the community
Partner closely with Lifestyle Consultants to support the sales process and new resident transitions
Provide same-day or timely assessments to support hospital discharges and urgent inquiries
Act as a clinical resource to help match residents to the appropriate community
Build and maintain relationships with hospital staff, discharge planners, and community partners
Travel within the region as required
Maintain accurate documentation and assessment records
