Make a difference As part of a very progressive Ambulatory Care pharmacy team at Community Health Network, you will participate in team-based care under collaborative drug therapy agreements where as a pharmacist you may make medication changes, as appropriate. Our ambulatory cardiology clinical pharmacy specialists are an integral part of the interprofessional team. As an ambulatory cardiology clinical pharmacy specialist, you will manage hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, anticoagulation service, as well as transitional care management for cardiology discharge patients both in-person and virtually. Other aspects of this position include providing staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement. You will have opportunities to mentor and teach pharmacy students and pharmacy residents along with serving as a clinical resource for the healthcare team. Your expertise in your field will allow you to advance the level of patient care to achieve the best possible outcomes. This position will provide patient care in General Cardiology Clinic, Ambulatory Home Blood Pressure Monitoring Program, Electrophysiology Clinic, Heart Failure Clinic and Anticoagulation Clinics. Expansion of cardiology pharmacist services to clinic sites in central Indiana and Indianapolis region will provide the pharmacist an opportunity to advance services with the support, resources, and infrastructure of a well-established ambulatory cardiology pharmacy team.
