Clinical Pharmacy Specialist, Ambulatory Care FT

Requirements
- Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience.
- PGY-1 Required
- Active New Jersey pharmacy license.
Nice To Haves
- PGY-2 Ambulatory Care Preferred.
Responsibilities
- Participates in patient rounds (or equivalent) with appropriate individuals including physicians, nurses and other health care providers.
- Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy.
- Drug Use Evaluations
- Provide education to patients/families, and other health care practitioners.
- Participate in medication safety initiatives
- Works within key sub-specialty areas to develop and maintain specific routine monitoring criteria for these patients.
- Represents the Pharmacy Dept. in system wide and divisional committees as needed.
