Position Summary: Revere Health is seeking a dedicated and compassionate Advanced Practice Practitioner (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join our dynamic Cardiology team. This role supports our mission to deliver high-quality, value-based care to patients with cardiovascular conditions.
Requirements
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Utah.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and teamwork abilities.
- Commitment to patient-centered, value-based care.
Nice To Haves
- Certification in cardiology or relevant clinical experience preferred.
Responsibilities
- Conduct comprehensive patient assessments, including history, physical exams, and diagnostic evaluations.
- Collaborate with cardiologists to develop and implement individualized treatment plans.
- Manage chronic cardiovascular conditions and provide follow-up care.
- Educate patients and families on disease prevention, treatment plans, and lifestyle modifications.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Document patient encounters accurately and timely in the EMR system.
