About The Position

Responsibilities include conducting and analyzing patient treadmill testing, performing comprehensive patient assessments, and developing and implementing treatment plans in collaboration with cardiologists. The provider will manage follow-up care for patients with various cardiac conditions, order and interpret diagnostic tests, and provide patient education on disease management, medications, lifestyle modifications, and preventive care. Additionally, the role requires coordinating care with other healthcare professionals, providing weekly care at Lutheran Hillside Village in Peoria, IL, participating in quality improvement and patient safety initiatives, and accurately documenting patient encounters in the electronic medical record.

Responsibilities

  • Evaluate, diagnose, and manage patients with acute and chronic cardiovascular conditions
  • Conduct, analyze and record results from patient treadmill testing
  • Conduct comprehensive patient assessments, including histories and physical examinations
  • Develop and implement treatment plans in collaboration with cardiologists
  • Manage follow-up care for patients with heart failure, coronary artery disease, arrhythmias, hypertension, and other cardiac conditions
  • Order, interpret, and review diagnostic tests and laboratory results
  • Provide patient education regarding disease management, medications, lifestyle modifications, and preventive care
  • Coordinate care with primary care providers, specialists, nursing staff, and ancillary departments
  • Provide weekly care at Lutheran Hillside Village in Peoria, IL
  • Participate in quality improvement and patient safety initiatives
  • Document patient encounters accurately and efficiently within the electronic medical record
