The Cardiac Transitional Care Advanced Practice Provider (NP or PA) role focuses on coordinating care for patients post-Acute Myocardial Infarction (AMI) to reduce readmissions . Key requirements include at least 3 years of acute care/hospital experience, with strong preference for cardiology, as well as proficiency in medication reconciliation, discharge planning, and patient education. The Advanced Practice Nurse (APN) or PA utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Schedule to be Monday to Friday either 8a-5p or 8:30a-5:30p or possible for (4) 10s schedule 7-5:30 pm between Mon- Friday, weekend coverage as needed, 1 holiday a year commitment, No weekends, No holidays. Location is for Ocean University Medical Center in Brick, NJ.
