About The Position

The Device Clinic Coordinator provides high quality patient care to the cardiac patient utilizing the nursing process and follows all established standards of practice within the State and Federal guidelines. The Device Clinic Coordinator will coordinate with other members of the Electrophysiology clinical team to effectively manage and support electrophysiology patients.

Requirements

• Active professional licensure in area of certification (i.e., LPN or RN).
• Minimum of three years clinical experience in cardiology including specific knowledge of cardiovascular device clinic management.
• Ability to work independently and manage time and tasks efficiently.
• Strong communication skills.

Responsibilities

• Coordinate and perform in person device checks including: Working with scheduling team to maximize clinic’s productivity and efficiency to meet the clinical needs of the practice, Performing device interrogations, programming, and troubleshooting, Coordinate programming adjustments as directed by providers, Schedule device reps when needed, Document device interrogations in the EMR and submit to provider for review, Provide appropriate patient education, Troubleshoot, determine urgency and escalation needs to cardiology/EP providers, Document all patient interactions and follow up in EMR
• Educate patients and their families about cardiac device management, including remote monitoring and lifestyle modifications.
• Prepare forms associated with devices including MRI clearances, return to work forms, etc.
• Perform post-procedure site checks including: Documentation of findings including photo in EMR, Communication of outcomes to providers and escalation when necessary
• Ensure electrophysiology patients are scheduled for appropriate cadence of remote and in person interrogations and maintain tracking system to allow follow up with non-compliant patients.
• Coordinate with remote device monitoring team to support patient education and follow up needs as necessary.
• Ensure clinic’s patient lists are accurately maintained with manufacturers and in Heart+ remote monitoring platform.
• Manage short term rhythm monitoring platforms, including holter monitors and MCOTs, by registering patients for testing, troubleshooting incomplete tests, providing patient education, triaging alerts and coordinating provider directed testing follow up.
• Actively participate in creating and implementing workflow improvements
• Respond to all telephone messages(patient, physician offices etc...) by end of shift
• Notify patients of test results (per physician instruction) and follow up
• Formulate and maintain effective working relationships with employees and other members of the health care team.
• Provide mentorship and education regarding device patient care to other clinic personnel
• Complete required continuous training and education, including practice specific requirements
• Demonstrate professional work behavior
• Comply with practice organizational policies and procedures, including maintaining patient privacy
• Perform other related duties as assigned or requested
