About The Position

Brentwood Hospital is looking for a dynamic Bridge to Discharge Counselor to join our team of professionals who are dedicated to providing help and hope to individuals and families for the achievement of improving their mental health. Help, by providing a safe, nurturing environment while they are with us; hope, by providing options for continued improvement when they leave this environment. The Bridge to Discharge Counselor provides individualized case management, psychotherapeutic services, and finalizes all discharge planing for adult psychiatric patients population (age range approximately 18 and up), adult CD patients population (age range approximately 18 and up), adolescent patient population (age range 13-17), child patient population, (age range approximately 4-11), and their families, spouse, significant other; to serve as a member of interdisciplinary team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment of adult psychiatric patients, adult cd patients, adolescent patients, child patients and their families.

Requirements

  • Master's Degree in social work, counseling or related field.
  • LPC or LCSW required
  • Masters Degree in social work, counseling or related field.
  • Demonstrated business development concepts, sales abilities, competency in computer usage, and excellent written and oral communication skills.
  • Knowledge of and skills in application of marketing and sales principles.
  • Creativity and flexibility.
  • Skills in data collection analysis and interpretation.
  • Record of adherence to deadlines.
  • Customer service skills.
  • Willingness to travel, including overnight travel as required.

Nice To Haves

  • Experience in inpatient or outpatient mental health setting preferred.
  • Prefer knowledge of the Louisiana behavioral health marketplace, insurance plans, target markets and referral sources, psychiatric and chemical dependency treatment and, age-specific programming.

Responsibilities

  • Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs.
  • Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet the patients' aftercare need.
